How to Apply for the Surviving Member Certificate Online?

Each state has a dedicated portal where you can apply for the surviving member certificate online. Please note that while the online application steps are generally consistent across portals, they may vary slightly due to differing state requirements. Residents of Delhi can apply online for a surviving member certificate through the e-District portal. Here's how you can apply:

  • Log in to the website: Log in to Delhi e-District Portal.
  • Upload the documents: Upload the required documents and photographs on the website.
  • Affidavit: You need to upload a scanned copy of your original self-declaration form.
  • Submit hard copy: You may submit the self-declaration form in person or via mail to the local SDM or Tehsildar office with the application or acknowledgement number.
  • Physical verification: Officials may call you for physical verification of documents.
  • Get the certificate: Once verified, an SMS or email will be sent to the registered mobile number or email ID. You may then proceed to the municipality or Tehsildar office for collecting the surviving member certificate.

How to Apply for the Surviving Member Certificate? - Offline Process

The below steps must be followed when applying for a surviving family member certificate offline:

Get the application form

Collect the application form from the local municipal office or Tehsildar.

Fill out the form

Provide the correct details while filling out and signing this form. The following information should be provided:

  • Full name of the applicant

Get the certificate
