- Step 1: Visit the official website of Passport Seva or any Passport Seva Kendra offline to procure 'Form number 2 for miscellaneous services’ and fill it up.
- Step 2: If you are filling out the form online, choose the ‘Re-issue of passport’ option.
- Step 3: Next, select the ‘Change in existing personal particular’ option and then select ‘Spouse name’.
- Step 4: Click on ‘Change in existing personal particulars’.
- Step 5: Fill the updated details and submit.
- Step 6: Pay the fee and book an appointment at the Passport Seva Kendra.
- Step 7: Visit the passport office with all your required documents.
Documents Required to Remove Spouse Name From Passport

- Application Form : The complete and filled out application form.
- Original Passport : Your original passport along with self-attested photocopies of its first and last two pages.
- Divorce Certificate (If Applicable) : Self-attested copy of the divorce certificate.
- Death Certificate (If Applicable) : Copy of the spouse's death certificate.
- Age Proof : Birth certificate, Aadhaar card, etc.
