How to renew?

Visit the policy renewal page on the website.
Enter policy number and personal details.
Check the renewal amount displayed.

Intimate the insurer before your hospitalisation or as soon after as possible.

What is Activ Assure Classic Health Insurance Plan?

A senior citizen insurance plan that supports your loved ones at every stage: when they are healthy, hospitalised, and recovering. From nursing at home to AYUSH cover to a dedicated, personal health coach, this plan offers the comprehensive care they deserve in their golden years.

How can you earn up to 21% of your premium as HealthReturns™ in the Activ Care Classic Plan?

Step 1 - Take the Health Assessment and get your Healthy Heart Score™.
Step 2 - Complete 13 Active Dayz™ per month and earn up to 20% HealthReturns™.
Step 3 - Earn an additional 1% by talking to your health coach.

How can you use HealthReturns™?

What are the permanent exclusions of the Activ Care Classic Plan?

Few of them are:-

Hearing aids, spectacles or contact lenses including optometric therapy, multifocal lens.
Treatment for baldness, alopecia, wigs, toupees, and all similar treatments.
Cosmetic, aesthetic and re-shaping treatments, and surgeries.

Any illness/injury/accident due to abuse of intoxicants, smoking cessation programs and the treatment of nicotine addiction, unless prescribed by a Medical Practitioner.
War, invasion, act of foreign enemy, uprising, revolution, insurrection, military or usurped acts.
Intentional self-injury or breach of law with criminal intent.

How can you earn one Active Dayz™?

Completing a fitness centre or yoga centre activity for a minimum of 30 minutes at our panel of fitness or yoga centres.

Recording of steps as per grid in a day (tracked through our mobile application or a wearable device linked to the Policy Number).

Burning 300 calories in one exercise session per day.

Participation in a recognised marathon/walkathon or a similar activity that offers a completion certificate with timing.

What is the free-look period for this plan?

If you cancel during the free look period (15 days for offline, 30 days for online), you will get your premium back minus the expense to the insurer.
Deductions may include expenses during coverage, stamp duty, and the number of insured days.
If cancelled after the free-look period, refunds are 75% within 1 month, 50% within 3 months, and 25% within 6 months, for the 1-year policy.
If cancelled after the free-look period, refunds are 85% within 1 month, 75% within 3 months, 60% within 6 months, 50% within 12 months, 30% within 15 months, and 20% within 18 months, for the 2-year policy.

What is the claims process?

Step 1 - Gather the required documents - the policy document, doctor’s note, bills, ID proof, etc.
Step 2 - Inform us about the claim event by phone, email, website, or by visiting our branch office.
Step 3 - Fill out and submit the claim forms along with all of the required documents.
Step 4 - Track your claim through our website or by contacting our customer support.
Step 5 - Once your claim is verified and approved, the settlement payout will be processed.

FAQs on Activ Care Classic Plan

All Insured Persons in this Plan may avail of a Health Assessment™ once in a Policy Year post successful Renewal of their Policy, for the following tests:

1. MER (Medical Examiner's Report stamped and signed) including BP (Blood Pressure), BMI (Body Mass Index), HWR (Hip Waist Ratio), and smoking status.

2. Blood sugar.

3. Total Cholesterol.

All Insured Persons may avail of a comprehensive health check-up once in a Policy Year on successful Renewal of the Policy, as specified in the Policy

Schedule / Product Benefit Table in accordance with the following list:

1. Complete Blood Count

2. C- reactive protein

3. Glycated haemoglobin test

4. Lipid profile

5. Liver profile

6. Kidney profile

7. Urine routine

8. Prostate-specific antigen for male

9. Electrocardiogram

10. S Electrolytes

The Insured Person’s test results will be used to calculate the Healthy Heart Score™ based on their completed Health Assessment™. The Healthy Heart Score™ will then be used to identify which category the Insured Person’s heart health falls in:

• Green: Low risk of heart disease compared to peers in the same age and gender group.

• Amber: Moderate risk of heart disease compared to peers in the same age and gender group – intervention will be beneficial.

• Red: High risk of heart disease compared to peers in the same age and gender group – immediate intervention is required.

The Healthy Heart Score™ is valid for 12 months, and will automatically be updated based on the latest available test result if another Health Assessment™ is completed.

For Healthy Heart Score™ to be calculated, Health Assessment™ needs to be carried out a minimum of once in a Policy Year.

All Insured Persons in the Policy may avail of an advance health check-up once in a Policy Year for the following list of tests:

1. Mammography

2. PAP smear

3. Thyroid function test

4. Treadmill Test (Two-Dimensional Echocardiography if the customer is not able to walk on the treadmill)

5. Chest X-ray

6. Sonography Abdomen

7. ENT check-up

8. OPG (Dental X-ray)

9. Vitamin D

10. Vitamin B12

11. Calcium

There are no documents required as such for purchasing a health insurance plan in India. You may only have to undergo a pre-policy medical check-up if you are a senior citizen. However, you must have a valid proof of your identity, address, age, etc. when you need to file a claim with your insurer.
